Physicians (Fellows and Residents) who trained in the University of Toronto, Department of Medical Imaging, may request verification of training within the department. These training verifications are for postgraduate medical education only. Undergraduate studies will not be verified by the department.
Please Note: Verifications for training that took place prior to the 80s can not be completed by the department as the records are at the hospital level.

Costs Associated with Training Verification
Applied fees for a physician (Fellow or Resident) who was registered in the Department of Medical Imaging within the last one (1) year will have their fees waived.
Applied fees for a physician (Fellow or Resident) who was registered in the Department of Medical Imaging within the past five (5) years will be $200 CAD.
Applied fees for a physician who was registered in the Department of Medical Imaging more than five (5) years will be $400 CAD.

Applied fees listed will cover all requests for a physician from a single requesting organization for one (1) year.
Timelines for Training Verification
Training verifications will not be processed until payment has been received and consent to release information has been signed by the physician and received by the Department of Medical Imaging.
Upon retrieval, training verifications will aim to be completed within 10–15 business days of receiving both payment and consent.
